Housing Stabilization Services are designed to support people with disabilities, mental illness, or other health conditions in finding and maintaining housing. These services help individuals transition to housing, sustain housing, and ensure they have the skills and resources needed for long-term housing stability.
To be eligible for Housing Stabilization Services, you must:
Be on Medical Assistance
Have a disability or disabling condition
Be 18 years or older
Be experiencing housing instability, such as being homeless, at risk of homelessness, or living in unstable housing situations.

Housing Stabilization Services include three main components:
Housing Consultation: Assistance in developing a housing-focused support plan.
Housing Transition: Help with finding and securing housing, including developing a housing support plan, identifying housing options, and navigating the application process.
Housing Sustaining: Ongoing support to maintain housing, including help with understanding lease requirements, resolving issues that could lead to housing instability, and connecting to community resources.
